Tip #1) Write Down Provider Information

As a patient, it is crucial to write down who is a part of your medical team. Your medical team is not only comprised of your primary care physician, but also includes your specialists as well. Keeping track of all the names, addresses and phone numbers of your providers is imperative to organizing your health.

Tip #2) Record Medical Conditions and Medications

There is a dedicated section in our Patient Medical Planner for you to record all your various health conditions and medications. Dr. Steven Ching, our internist who practices out of our Hemet location, loves when new patients come in with a full and detailed record so that he can have a better understanding of your overall health.

Tip #3) Take Notes During Appointments

Keeping track of what was talked through during appointments is crucial to managing your medical care, especially when you have multiple specialists. As much as specialists try to keep track of everything, we want to empower our patients to have their hands in their own medical care.

Tip #4) Keep All Emergency Contacts and Doctors All in One Place

We realize there may be more than one person aside from you involved in your medical care. This could be a spouse, a child, or a caregiver that you would like to have access to your care. In our RFMG Patient Medical Planner, there is a dedicated place for you to write down all the people involved in your care.

5) Keep Track of All Your Most Important Questions for Your Doctors

Before going to your appointments, we encourage you to write down the most important topics that you want to address with your doctor. We want to take care of every need you have, but it is more important to go deeper into a few issues than shallowly address many. Prioritizing what issues you want to bring to each appointment will give you a better experience and a more thoughtful medical plan moving forward.
