Commercial HMO
1. Call your member service number on your insurance card.
2. Ask if your plan includes SEAVIEW IPA as the HMO provider.
3. You can request a change from the other IPA's to Seaview
4. Request one of our physicians. Your insurance will send you a new card once confirmed.
Commercial PPO
1. Verify you are "in-network" to avoid higher costs. Call the service number on your insurance card.
